Flexpoint Ford has formed Corva LLC, a platform focused on acquiring, developing, and operating dialysis clinics. It will be led by Jerry Simonsen, former CEO of Renal Management Inc., and already has acquired Innovative Renal Care – a Houston, Texas-based dialysis center for those with end-stage renal disease. Industrial Acoustics Company Inc., a portfolio company of AEA Investors, has acquired GT Exhaust Inc. from CID Capital. No financial terms were disclosed. GT Exhaust is a Lincoln, Neb.-based provider of sound and emissions control solutions to the North American energy, power and heavy transportation industries. Lincoln International managed the sale process. www.gtexhaust.com

GTCR has acquired OneSource Information Services, a Concord, Mass.-based provider of online business information and sales intelligence solutions, from Infogroup Inc. No financial terms were disclosed. The deal was done via Cannondale Holdings, a data-enabled information and marketing services platform GTCR formed earlier this year with a $300 million equity commitment. www.gtcr.com

Morgan Stanley Global Private Equity has acquired a majority stake in Creative Circle LLC, a North American professional staffing firm focused on advertising, creative and marketing talent. No financial terms were disclosed. Sellers include Riordan, Lewis & Haden Equity Partners. www.creativecircle.com

Permira is the frontrunner to acquire genealogy website Ancestry.com (Nasdaq: ACOM), according to Reuters. Other suitors include Hellman & Friedman and TPG Capital, in a deal that could be valued at more than $1.5 billion. www.ancestry.com

Platinum Equity has agreed to acquire BWAY, an Atlanta-based packaging company, from Madison Dearborn Partners for $1.24 billion. www.bwaycorp.com

Pyramid Healthcare Inc., a portfolio company of Clearview Capital, has acquired High Focus Centers, a Summit, N.J.-based provider of structured outpatient substance abuse and psychiatric treatment programs. No financial terms were disclosed. www.clearviewcap.com

Supervalu Inc. (NYSE: SVU) has received buyout interest from KKR, TPG Capital and Ron Burkle, according to Bloomberg. Cerberus Capital Management also has examined certain Supervalue assets. The grocery chain recently said it was exploring strategic options, and has a market cap just south of $500 million. www.supervalu.com

Waterland Private Equity has narrowed the bidders for Dutch trust and corporate management business Intertrust Group down to five, according to Reuters. They are: The Blackstone Group, The Carlyle Group, Cinven, Goldman Sachs Private Equity and Pamplona. Final bids are due by November 7, with ING managing the process.
